What is Breast augmentation?

Breast augmentation is a surgical procedure to increase breast volume, for a harmonious body shape. At Ästhetik am Isartor in Munich, under the medical direction of Dr. Fabian Weinschenk, we offer two methods: augmentation with CE-certified silicone implants — established, with a manufacturer's lifetime warranty, round or anatomical (teardrop) — and fat transfer (autologous fat grafting), in which the body's own fat from the abdomen, hips or thighs is gently aspirated, purified and re-injected into the breast tissue. Implants allow a marked volume increase of one to three cup sizes, via three accesses (submammary in the inframammary fold, periareolar around the areola, axillary in the armpit) and two positions (sub-pectoral beneath the chest muscle, or sub-glandular above it). Fat transfer stays scar-sparing and allows a gentle enlargement of about half to a full cup size per session, without foreign material. The ability to breastfeed usually remains: a properly performed operation preserves the glandular breast tissue and the milk ducts. The operation takes place under general anaesthesia, lasts one to two hours with implants and two to three hours with fat transfer (incl. the harvest), and requires one night as an inpatient. Who is Breast augmentation suitable for?

Anyone considering a breast augmentation has as a rule thought about it for years. Breast augmentation corrects a breast volume that is too small or a congenital asymmetry — in two methods, depending on the starting findings.

Suitable for

Breast augmentation is suitable where there is an aesthetic wish for more volume, a congenital asymmetry, a loss of volume after pregnancy or breastfeeding, or an age-related loss of volume. Implants suit a clear wish for 1–3 cup sizes more volume; fat transfer suits a gentle enlargement of ½ to a full cup size per session (repeated in a second session for more volume where needed), a wish for a scar-sparing solution without foreign material and sufficient donor tissue (abdomen, hips, thighs). The requirements: a stable phase of life, completed family planning recommended but not essential (the ability to breastfeed usually remains), realistic expectations.

Not suitable for

Breast augmentation is not advisable where there are breast findings that have not been clarified — from the age of 35, mammography or breast ultrasound is a requirement before the procedure, and with fat transfer it also serves as a baseline for the follow-up checks. Acute wound-healing disorders or infections and active anticoagulation that cannot be paused postpone the procedure. Nicotine use should be paused for at least four weeks.

The choice between implants and fat transfer depends on how much volume you would like, how much donor tissue is available and which phase of life you are planning for.

With both methods the glandular breast tissue is preserved — the ability to breastfeed usually remains. The indication is established in the medical initial consultation (€100). Clinic fees are shown separately.

Shape

Round or anatomical. Sub-pectoral or above it.

With the implant method, two further variables decide the shape: round (a classic look, more upper-pole volume) or anatomical, teardrop-shaped (modelled on the natural breast) — and the position, sub-pectoral (beneath the chest muscle, a more natural transition) or sub-glandular (above it, a shorter recovery time). We use exclusively CE-certified implants from established manufacturers with a lifetime warranty.

How the procedure works

Four steps, clearly separated — from the initial consultation with a sizer-bra fitting, through thorough pre-operative preparation, to six weeks of aftercare with a compression bra.

    Preparation

    The pre-operative tests (blood count, ECG, mammography / breast ultrasound from the age of 35) must be available. No nicotine for at least 4 weeks beforehand.

    Operation

    The operation is carried out under general anaesthesia. With implants: the incision is submammary, periareolar or axillary, and the implant position sub-pectoral or sub-glandular — duration 1–2 hours. With fat transfer: first a gentle micro-liposuction at the donor tissue, then purification and re-injection into the breast tissue — duration 2–3 hours, including the fat harvest.

    An inpatient stay of 1 night. Compression bra for 6 weeks, day and night — it shapes the breast afterwards and supports healing. Avoid heavy physical work and sport for 6 weeks; desk work is possible again after 3–7 days. First check-up after 1 week, further ones after 6 weeks and 6 months.

Recovery

Recovery & final result

  1. DAY

    0–1.

    Waking up, resting.

    Operation; the compression bra is fitted straight away. 1 night of inpatient observation. Pain is usually mild to moderate — well managed with painkillers.

  2. DAY

    2–7.

    Button at the front.

    Strict rest. Compression bra around the clock. No movements above the head, no lifting.

  3. WEEK

    2–6.

    Step by step.

    Desk work is possible again after 3–7 days. Compression bra still day and night. Gentle walks are allowed; no movements above the head, no sport.

  4. WEEK

    6–12.

    Sport returns.

    The compression bra ends after 6 weeks — after that, a sports bra for sporting exertion. Sport is resumed step by step. With implants: the shape stabilises.

  5. MONTH

    3–12.

    The final result.

    With implants: the final result is visible after 3–6 months, with the implant naturally integrated. With fat transfer: the definitive shape follows once the resorption has subsided (3–4 months); where more volume is wanted, 2–3 staggered sessions are possible.

The ability to breastfeed remains possible after both methods in most cases — when the operation is carried out properly, the glandular breast tissue and the milk ducts are preserved. Tops that open easily at the front are recommended.

Wearing a compression bra for six weeks, day and night, is not a mere recommendation — it actively supports the shape you would like. In the first few days it is best to reach only for tops with a button placket; please do without jumpers or T-shirts that have to be pulled over the head for the time being. That is part of the routine and helps your body to adapt as well as possible.

Risks

Risks & informed consent

Carried out properly, breast augmentation is an established procedure. Risks are documented in detail in the personal informed-consent consultation in accordance with § 8 HWG (German Health Advertising Act).

  1. General

    Swelling and haematoma at the breast are part of the expected course in the first weeks after the procedure and typically subside within a few weeks under consistent compression. Wound-healing disorders are reduced to a minimum by sterile operating conditions, a consistently worn compression bra and strict rest in the first few days.

  2. Specific

    With the implant method: capsular contracture (a hardening of the connective-tissue capsule around the implant) is rare with modern CE-certified implants and is decreasing further; displacement of the implant is minimised by precise pre-operative planning. — With fat transfer: the resorption rate of 30–50 % over 3–4 months is part of the method, not a complication — the body breaks part of the transferred fat down again, and the material that remains stays permanently. Micro-calcifications or micro-cysts can form and become identifiable on mammograms — baseline imaging before the procedure is therefore recommended. — Possible with both methods: a temporary change of sensation at the nipple, which as a rule resolves within months.

  3. Very rare

    Thromboembolic events are avoided by early mobilisation, consistent compression and, where appropriate, heparin prophylaxis. A routine implant exchange is not required; in rare cases — where there is a material defect or at the patient's request — an exchange after years can make sense.

    The choice depends on three factors.     Both shapes are established and are chosen individually. Round: a classic look with more upper-pole volume, giving a fuller décolletage line. Anatomical (teardrop-shaped): modelled on the natural breast, giving a softer shape that looks natural. In the initial consultation we fit different sizer bras so that shape and volume can be compared visually.

  • Where does the incision sit?

    Three accesses are possible. Submammary: in the inframammary fold — the standard access, with a scar that is barely visible once healed and with precise implant positioning. Periareolar: at the transition from the areola to the breast skin — the scar is well hidden, and a slight effect on sensation is theoretically possible. Axillary: in the armpit fold — no scar on the breast itself, positioning technically more demanding. Which access comes into question we decide in the initial consultation.

  • Sub-pectoral or sub-glandular?

    Sub-pectoral (beneath the chest muscle) gives a more natural transition, is the standard where breast tissue is slim, and needs a longer recovery time. Sub-glandular (above the muscle, beneath the glandular breast tissue) has a shorter recovery time and suits denser breast tissue — the risk of visible implant edges is higher where the starting breast is very slim. The decision is individual.

  • When can I do sport again?

    Sporting activity is possible again after 6 weeks, step by step. You wear the compression bra for 6 weeks, day and night. Desk work is possible again after 3–7 days; avoid movements above the head and lifting in the first 2 weeks. In the first few days wear tops with a button placket (do not pull a T-shirt over your head).
