Botulinum toxin type A has been used medically for over 30 years and is safe when dosed properly by a medically qualified practitioner. We use exclusively CE-certified preparations licensed in Germany. Risks (temporary bruising, swelling and, very rarely, slight eyelid or brow drooping) and the mandatory active-substance information under § 8 HWG are documented in the personal informed-consent consultation. Use in pregnancy, while breastfeeding and in neurological conditions is contraindicated.

The effect lasts three to six months on average. With repeated use at regular intervals, many patients find in practice that the duration of effect becomes longer. A top-up is typically needed once or twice a year; when it makes sense is something we discuss individually at the follow-up appointment.

Six typical treatment zones: forehead lines (horizontal, m. frontalis), the glabellar line (frown line between the eyebrows, m. corrugator + procerus), crow's feet (periorbital, m. orbicularis oculi), bunny lines (fine lines on the bridge of the nose, m. nasalis), chin lines (pebble chin, m. mentalis) and the masseter muscle (jaw slimming and bruxism therapy). Which zone is an option is something we decide in the initial consultation.

The so-called lip flip is a minimally invasive botulinum toxin treatment that reshapes the upper lip subtly, without adding volume. Relaxing small parts of the muscle around the upper lip in a targeted way turns it slightly outwards. The upper lip then looks fuller and the vermilion more visible, especially at rest and when smiling. It suits a fine emphasis of the lip contour, and an upper lip that rolls inwards. The effect sets in within a few days and as a rule lasts three to four months.

Local reactions (redness, small bruises at the injection sites, a brief feeling of pressure) are typical and fade within 1-3 days. Rarely: temporary slight eyelid or brow drooping where the injection is not precise (this resolves completely as the effect wears off). Very rarely: headache for 1-2 days after a first treatment. Onset of effect after seven to ten days. The full final result after approximately 14 days. The change is subtle and gradual. If fine-tuning makes sense after 14 days, a follow-up treatment is possible.

On the day of treatment, please no sport, no sauna and no alcohol — a 24-hour pause. From day 1 normal sporting activity is possible again. For 4 hours directly after the treatment please stay upright (no lying down, no massage of the treated zone).

Botox (botulinum toxin) relaxes the muscles of facial expression — suitable for dynamic lines that form in movement (forehead, glabella, crow's feet). Hyaluronic acid fillers fill volume deficits — suitable for static lines and loss of volume in the cheeks, the tear trough or the lips. Where the upper lip is long and the vermilion thin, a bullhorn lift can also make sense. We discuss the right choice in the initial consultation.

Botox is contraindicated in pregnancy, while breastfeeding, in neuromuscular disorders (e.g. myasthenia gravis, ALS), where there is a known allergy to the active substance, in acute infections in the treatment area and where there is active anticoagulation that cannot be paused. For purely static lines without a dynamic component, hyaluronic acid is the right method. In the initial consultation we clarify pre-existing conditions and the indication individually.

Baby Botox is a low-dose, preventive application using roughly 30-50% of the standard dose. The aim: a discreet smoothing of emerging expression lines without restricting movement — natural facial expression stays visible. It suits younger adults from their mid-20s who want to prevent first expression lines, or as a discreet first treatment. Procedure, onset of effect and aftercare are the same as for the regular treatment. For two reasons — one legal, one ethical. First, the German Heilmittelwerbegesetz (§ 11 HWG, the Health Advertising Act) prohibits advertising surgical plastic procedures with before-and-after photographs on publicly accessible websites — a rule in force since 2012 that protects patients from distorted promotional depictions. Second, we respect our patients' privacy: even where written consent exists, many would rather intimate photographs of their body did not circulate on the open web. Those images are shown with the written consent of the patients concerned, exclusively within the protected setting of the consultation, and are never published online. Three objective criteria help you get your bearings. First, the specialist qualification: "Facharzt für Plastische und Ästhetische Chirurgie" is a legally protected German specialist title requiring several years of specialist training — unlike general, unprotected descriptions such as "cosmetic surgeon". Second, membership of a medical professional society such as the DGPRÄC (the German Society of Plastic, Reconstructive and Aesthetic Surgeons), which requires binding professional standards and continuous further training. Third, the quality of the consultation: a reputable practice takes its time, is honest about risks, alternatives and realistically achievable results, and does not push you towards a decision.
