What is Mommy Makeover?

The Mommy Makeover is a co-ordinated arc of treatment combining several plastic and aesthetic procedures, addressing the typical physical changes after pregnancy and breastfeeding. At the Ästhetik am Isartor practice in Munich, under the medical direction of Dr. Fabian Weinschenk, we bundle three components matched to one another: a tummy tuck with, where needed, restoration of the abdominal muscles (correction of a rectus diastasis), breast surgery (a lift, an augmentation with implants or fat transfer, or a reduction) and liposuction of the hips, outer-thigh fat pads or inner thighs. Depending on the findings, the procedures are combined in one session or staggered over time. Preconditions: completed family planning, a weight stable for at least six months, at least six to twelve months since the last birth, and non-smoker status or nicotine abstinence of at least four weeks. The operation takes place under general anaesthesia with one to three nights as an inpatient; in a combined session the surgery takes three to seven hours. Recovery: two weeks taking it easy, six weeks of compression with a compression bra and abdominal binder, full sporting activity after six to eight weeks, and the final result assessable after three to six months.

A Mommy Makeover bundles several plastic and aesthetic procedures — each individual component is subject to its own medical indication and informed-consent consultation under § 8 HWG (German Health Advertising Act). We use exclusively CE-certified materials approved in Germany (breast implants, fillers, hyaluronidase as an antidote). For legal reasons we carry no brand advertising on the website. Suitability

Who is Mommy Makeover suitable for?

Pregnancy, birth and breastfeeding change the body fundamentally. Typical changes — lax skin on the abdominal wall, a rectus diastasis, loss of volume and laxity of the breast, stubborn fat pads on the hips and outer thighs — often cannot be corrected by exercise and diet, because they are structural. A Mommy Makeover bundles several plastic-surgical procedures into a co-ordinated arc of treatment that addresses these changes specifically.

Suitable for

A Mommy Makeover is suitable with completed family planning, with a weight that has been stable for at least 6 months, with lax skin on the abdominal wall with or without a rectus diastasis, with loss of volume or laxity of the breast after breastfeeding, with stubborn fat pads on the hips, outer thighs or inner thighs that do not respond to exercise and diet, with a minimum interval of 6-12 months since the last birth and a break from breastfeeding of at least 3 months, and with a general condition sufficient for longer operating times and the strain of anaesthesia. Precondition: nicotine abstinence for at least 4 weeks beforehand and afterwards (critical for wound healing at the T-junctions of the tummy tuck).

Not suitable for

A Mommy Makeover is not appropriate with family planning that is not complete (a further pregnancy would markedly impair the result of the tummy tuck), with an unstable weight or a further planned weight loss of more than 5 kg, with less than 6 months since the last birth or breastfeeding that is still going on, with acute illnesses or underlying conditions that are difficult to control, with active nicotine use without a willingness to abstain, with a raised thromboembolic risk that cannot be addressed prophylactically, and with a BMI > 30, where a tummy tuck is as a rule associated with a raised risk of wound-healing problems (weight reduction recommended first).

Method

Three building blocks. One co-ordinated plan.

The Mommy Makeover is not a single procedure — it is a bundle of three (sometimes four) building blocks that are chosen according to the findings and then either combined in one session or staggered over time. Each component has its own indication, operating time and recovery — the table below gives an overview.

Planning

Bundling or staggering — the decision is a medical one.

Where the starting point is moderate and the general condition good, we combine several components in one session (3-5 hours of operating time). The advantage: a shared recovery, one anaesthetic, a single inpatient admission. With more extensive procedures or a raised risk profile we spread the procedures over 2-3 sessions across several months. The advantage: shorter individual operations, less strain from anaesthesia, a staggered recovery. Safety has priority — the plan is set medically in the initial consultation, after a clinical examination and, where appropriate, an additional assessment in internal medicine.

Four steps across several months — from the medical initial consultation with the choice of components, through preparation and the day of surgery with 1-3 nights as an inpatient, to the six-week recovery with compression around the clock.

    Initial consultation & bundle planning

    In the personal initial consultation (€100) we clarify what you would like, the initial findings (abdominal wall, breast, fat pads), your family planning, the stability of your weight and your general health. We mark the planned components on paper (a three-zone sketch with a peach ellipse for the breast, a horizontal line for the W-shaped tummy-tuck incision and two outward-curving lines for the hip / outer-thigh liposuction) and decide medically whether the procedures are combined in one session or staggered.     Preparation

    Full blood count and ECG. Mammography / ultrasound recommended from the age of 35. Nicotine abstinence for at least 4 weeks before the operation. No blood-thinning medication in the 14 days before the operation (after consulting your GP). Organise the recovery garments in advance — a compression bra and abdominal binder for 6 weeks, and with additional liposuction of the inner thighs also compression tights for 4-6 weeks. Make sure of support from the family for the first 2-3 weeks (a babysitter or family help for heavy tasks).

    Day of surgery

    Inpatient admission on the day of surgery or the day before. General anaesthesia. Marking of the bundle zones with an ergonomic marker. In a combined session: a tummy tuck with correction of the diastasis (W-shaped incision), breast surgery of your choice, liposuction of 1-2 areas — 3-5 hours of operating time where the extent is moderate, up to 7 hours with more extensive procedures. Recovery

  1. DAY

    0–3.

    Inpatient. Thrombosis prophylaxis.

    Inpatient stay of 1-3 nights with close monitoring, pain management, thrombosis prophylaxis (low-molecular-weight heparin) and early mobilisation. The compression bra and abdominal binder are already fitted in hospital and worn continuously.

  2. WEEK

    1–2.

    Take it easy. Compression 24/7.

    Taking it easy at home. Compression garments around the clock. Light mobilisation: short walks, no heavy lifting (not the baby either — organise help). Outpatient wound checks after 5-7 days and after 10-14 days, with stitches or clips removed according to the findings.

  3. WEEK

    3–6.

    Getting back in.

    Light office work possible from week 2. Compression continues around the clock until week 6. Lift the baby carefully — we discuss lifting technique from a high chair or buggy in the informed-consent consultation. Wound healing at the T-junctions of the tummy tuck progresses; from week 4 a silicone gel sheet on the scar is typical.

  4. WEEK

    6–8.

    Sport returns.

    Where healing is good, compression can be loosened at night. Resume sporting activity step by step: light cardio from week 6-8, strength training from week 10-12. Sauna from week 8. Heavy physical work from week 8-10.

  5. MONTH

    3–6.

    Scar + final result.

    Intensive scar care for 6-12 months. The final result can be assessed after 3-6 months (swelling subsiding, the scar maturing, the final contouring). With breast surgery: implants settle into the chosen pocket position; with fat transfer part of the injected tissue is resorbed — the final volume is stable after 3 months.

Final result after 3–6 months — scar care for 6–12 months. Check-ups after 3 and 12 months.

Lifting the baby is the central challenge in the first 6 weeks — we discuss practical lifting techniques from a high chair, buggy and bed in the informed-consent consultation, and recommend organising family or a babysitter in advance for the first 2-3 weeks. No procedure is "urgent" enough to put safety at risk — we decide medically whether to operate in a combined or a staggered way, on the basis of the findings and the general condition.

Risks

Risks & informed consent

With combined procedures the operating time and the strain of anaesthesia are higher than with separate appointments — we take that into account when establishing the indication. Specific risks are documented in detail in the informed-consent consultation under § 8 HWG, for each component individually and for the combination together.

  1. General

    Swelling, haematomas, tenderness to pressure and temporary changes of sensation in the operated area are the typical post-operative courses. They subside completely within the first 2-6 weeks; temporary changes of sensation can last for months, but usually resolve completely. Scarring at the T-junctions of the tummy tuck and at the incisions of the breast surgery is supported by a silicone gel sheet from week 4 and by consistent sun protection.

  2. Specific

    Delayed wound healing at the T-junctions of the tummy tuck is the most specific risk — critically increased by active nicotine use, which is why nicotine abstinence of at least 4 weeks before and after the operation is required. Seroma formation (a collection of wound fluid) in the liposuction or tummy-tuck areas is possible and can easily be aspirated as an outpatient. Asymmetry after the breast or liposuction component can be adjusted where needed in a smaller follow-up correction after 6-12 months.

  3. Very rare

    Thromboembolic events are more likely with a longer combined operating time — we minimise this risk with low-molecular-weight heparin as thrombosis prophylaxis, with compression garments and with early mobilisation on the day of surgery. Anaesthetic complications are more likely with combined procedures because of the longer time under anaesthesia, which is why the indication is established medically, where appropriate with an additional assessment in internal medicine before the operation.   • Can I still breastfeed after a Mommy Makeover?

    With a breast lift or reduction the function of the mammary gland can be affected — which is why the precondition of "completed family planning" matters. With a tummy tuck and liposuction alone, the ability to breastfeed is not affected.   • Do I have to be smoke-free?

    Nicotine abstinence of at least 4 weeks before and 4 weeks after the operation is required — nicotine considerably impairs wound healing at the T-junctions of the tummy tuck and raises the risk of necrosis. With active nicotine use and no willingness to abstain, we do not operate. Where useful, we support patients with information about smoking-cessation programmes.

  • When is a Mommy Makeover NOT suitable?

    Contraindications: family planning that is not complete (a further pregnancy would markedly impair the result of the tummy tuck), an unstable weight or a further planned weight loss of more than 5 kg, less than 6 months since the last birth, breastfeeding that is still going on, acute illnesses or underlying conditions that are difficult to control, active nicotine use without a willingness to abstain, and a BMI > 30 (weight reduction recommended first).   • Why don't you show before-and-after photographs on your website?

    For two reasons — one legal, one ethical. First, the German Heilmittelwerbegesetz (§ 11 HWG, the Health Advertising Act) prohibits advertising surgical plastic procedures with before-and-after photographs on publicly accessible websites — a rule in force since 2012 that protects patients from distorted promotional depictions. Second, we respect our patients' privacy: even where written consent exists, many would rather intimate photographs of their body did not circulate on the open web. What you can expect instead: in the personal initial consultation (€100) we show you anonymised examples from our own practice documentation — matched to your indication, your build and your aesthetic goal. Those images are shown with the written consent of the patients concerned, exclusively within the protected setting of the consultation, and are never published online. You get a realistic sense of what to expect without anyone having to give up their privacy for someone else's advertising.

  • What should I look for when choosing a doctor for aesthetic plastic surgery in Munich?

    Three objective criteria help you get your bearings. First, the specialist qualification: "Facharzt für Plastische und Ästhetische Chirurgie" is a legally protected German specialist title requiring several years of specialist training — unlike general, unprotected descriptions such as "cosmetic surgeon". Second, membership of a medical professional society such as the DGPRÄC (the German Society of Plastic, Reconstructive and Aesthetic Surgeons), which requires binding professional standards and continuous further training.
